- Click on “Birth Details” button.
- Click on ” Apply Birth Registration” option under the Birth Details Section.
- A new Birth Registration Form will open on your screen.
- Select the District and Town Panchayat from the drop down.
- Enter your Valid Mobile Number and Email ID.
- Enter the Child Information like DOB, Gender and name.
